SNOVALLEY CULTURAL AND SPORTS CLUB, SNOQUALMIE, PARTICIPANT AND ARBITRATION AGREEMENT, INDEMNIFICATION, GENERAL RELEASE AND ASSUMPTION
I/we realize that participation in group dance and activities could involve some possible personal injury.Despite precautions, accidents and injuries may occur. By signing this waiver form, I/we assume all risks related to the use of any and all spaces used by Snovalley Cultural and Sports Club, Snoqualmie.I/we agree to release and hold harmless Snovalley Cultural and Sports Club, Snoqualmie including its volunteers, and facilities used by both entities from any cause of action, claims, or demands now and in the future.I/we will not hold Snovalley Cultural and Sports Club, Snoqualmie liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes.Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by Snovalley Cultural and Sports Club, Snoqualmie.
BY SIGNING IT, I/WE ARE GIVING UP MINE AND/OR MY SPOUSE AND MINOR’S LEGAL RIGHTS INCLUDING THE RIGHT TO BRING A LAWSUIT IN COURT AND/OR HAVE THE CLAIM DECIDED BY A JURY.
BY SIGNING THIS AGREEMENT, I/WE ARE GIVING UP MY RIGHTS AND THE RIGHTS OF MY SPOUSE AND/OR CHILD(REN) TO SUE SNOVALLEY CULTURAL AND SPORTS CLUB, SNOQUALMIE FOR ANY INJURY, INCLUDING PARALYSIS OR DEATH, CAUSED IN WHOLE OR IN PART BY THE NEGLIGENCE OR FAULT OF SNOVALLEY CULTURAL AND SPORTS CLUB, SNOQUALMIE, INCLUDING ANY OF ITS VOLUNTEERS, AFFILIATES, EQUIPMENT AND FACILITY SUPPLIERS.
Photographs and Videos Consent
I/we understand and agree that my photographs and videos taken during SnoDandiya'25 shall become the property of Snovalley Cultural and Sports Club, Snoqualmie and may be used for promotions.
I/we hereby affirm that such release to the Snovalley Cultural and Sports Club, Snoqualmie does not constitute any form of compensation, including royalties arising from the photographs and videos, to our benefit.
I/we hereby waive our right to inspect or approve the photographs and videos by which our likeness appears.
I/we hereby hold harmless, release, and forever discharge Snovalley Cultural and Sports Club, Snoqualmie from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on our behalf or on behalf of our estate have or may have by reason of this authorization.
